Why it happens
- The user's password reached its expiration policy limit.
- A sign-in or token refresh occurred after the password had already expired.
- The account requires a password reset before further access is allowed.
How to fix it
- Reset the user's password or allow the user to complete the password reset flow.
- After the password is updated, sign in again and refresh affected sessions.
- If this is unexpected, review password expiration policy for the account.
